Info Used for Costuming & Program Photos. Photo upload is optional. If you do not submit a photo, we will use the photo provided during audition submission.
When you submit, the form will go blank and you will see a small confirmation 
message on the bottom of the page. You will also receive a confirmation email.
Please note, once costume design is complete, cast members may be requested to bring base pieces of their own clothes. Artistic team will provide direction on this based on character and scene.
